How to Apply for Medicaid-Waiver Services

Are you in need of home care services but wondering where to begin? Beginning to take action can seem like a daunting task, but we are here to help. You may be eligible tot receive home care through Community-Based Waiver Services. If you're on Medicaid, you may be eligible to receive waiver services. If you are not on Medicaid, we can help you discover if you are eligible.

To begin this process with Blessings4Ever Home Care Agency, give us a call at (215)425-3950 so that we can assist you in filling out your application to the PA Independent Enrollment Broker. The "Steps to Apply" document below will give you details as to next steps after we submit your application.

Once you are completed with the process, you will be given the option to select a home care agency through your selected Managed Care Organization. Simply tell them that you choose Blessings4Ever Home Care Agency as your choice to receive home care services, and your coordinator will send us the authorization for services.

Once we receive your authorization, we can begin!
